Your document checklist
This form is for your own use. Completing it doesn’t send anything to me.
Before we can work out a fair financial arrangement, we need a clear picture of everything you both own and owe. This checklist mirrors the information required for Form E, the financial disclosure form used by the courts. Mediation doesn’t require you to complete Form E itself, but gathering the same documents ensures that the figures we work with are accurate and reliable.
You can work through the sections in any order and only complete the parts that apply to you — most people won’t need every category. Nothing needs to be perfect by your first session; please complete it as fully and accurately as you can, and we’ll identify anything still missing together.
Income from employment
If you’re employed (skip this if you’re only self-employed).
Ask payroll or HR, or check your employer’s online payslip portal.
Self-employment or a business
If you’re self-employed, a director, or a partner in a business.
Your accountant can usually turn these around quickly if you ask.
Bank and building society accounts
Most banking apps let you download or email statements as a PDF in seconds — often the quickest one to tick off.
Investments and savings
Check post or email from providers you may have set aside without opening.
Property
The family home and any other property either of you owns.
An online “instant estimate” isn’t precise enough on its own — most local agents will give you a free valuation letter if you ask.
Pensions
Every pension counts, including small or old workplace ones you’ve lost track of.
This is the one to request first — providers can take 4–6 weeks. If it hasn’t arrived by your session, bring the date you asked and when they said to expect it.
Life insurance and investment policies
Your policy documents will say whether it has a cash-in value; if you’re not sure, ask the provider.
Debts and money you owe
A recent statement or your online banking app will show the current balance.
